I understand that I participate in these activities at my own risk, and there shall be no liability whatsoever attached to ArcelorMittal Dofasco, its servants, agents, employees, other participants, or independent contractors for any loss, damage or injury of any nature, however caused. In case of any medical or other emergency that may occur while I participating in these activities, I hereby authorize ArcelorMittal Dofasco, its servants and employees, to take such action as they consider appropriate, including, without limitation, providing or obtaining medical or surgical attention.
I agree to comply with all municipal, provincial and federal requirements and protocols that may be in place from time to time in connection with Covid-19, together with any related requirements and protocols established by the ArcelorMittal Dofasco Retiree Club from time to time, and any failure to do so may result in termination of membership at the option of the ArcelorMittal Dofasco Retiree Club.

THE UNDERSIGNED PARTICIPANTS UNDERSTAND AND AGREE:
Activity may continue if the employee resigns from ArcelorMittal Dofasco and signs an appropriate release form. League/Club eligibility rules to continue to play must be adhered to.
Can have all privileges revoked immediately, at any time, at ArcelorMittal Dofasco’s sole discretion.
I am responsible for any damage I may cause to the facilities and I understand that I will be billed for the repairs.
In addition, pursuant to the Personal Information and Protection of Privacy Act, I consent to the use by ArcelorMittal Dofasco of my personal information for the purpose of the administration of the Facility’s programs and leagues and any disclosure to authorities, including but not limited to Hamilton Police Services, in connection with any investigations relating to the Facility and/or the Activity.
